JOB SUMMARY
Provide comprehensive physical therapy services to patients in a rehabilitation and skilled nursing facility setting. Evaluate patients, develop and implement individualized treatment plans, and educate patients and their families to improve function and quality of life.

Key Responsibilities
- Assess patient's physical condition, functional limitations, and goals through standardized tests and measures.
- Develop and implement individualized treatment plans based on assessment findings and physician's referral.
- Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and patient education.
- Monitor patient progress, re-evaluate treatment plans as needed, and document all findings and interventions accurately and timely.
- Collaborate with interdisciplinary healthcare team members to ensure continuity of care.
- Educate patients and their families on home exercise programs and strategies to maintain functional gains.
- Maintain productivity standards and adhere to all facility policies and procedures.

Experience
- Previous experience in a skilled nursing or rehabilitation setting is preferred.

Required Qualifications
- Graduation from an accredited Physical Therapy program with a Doctor of Physical Therapy (DPT) degree or Master's degree in Physical Therapy.
- State licensure as a Physical Therapist in Wyoming.

Certifications
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.

- Strong manual therapy and exercise prescription skills.
- Excellent communication, interpersonal, and documentation skills.
- Ability to work independently and as part of a team.
- Proficient in evaluating and treating a wide range of orthopedic, neurological, and general deconditioning diagnoses.
