  • Job Summary
    The Community Health Worker II (CHW II) at Client will primarily work at serving and assisting our patients while located at different sites throughout the organization and community events. (CHW II) will work at different capacities based on their assigned area of training and specialty. The CHW II will be a trusted member of the community, and actively participate as a liaison between primary care, specialties and resources available to Client patients. Activities may include, but are not limited to, outreach, engagement, care coordination, case management, linkage of the patient with resources at Client and in the community such as food, housing, financial and transportation assistance and also linking the patient to care within the clinic.

Under the general direction of the supervisor/manager, the Community Health Worker performs a variety of services.

    Duties include:
    1. CHW I tasks, plus:
    2. Completion of CEC certification to determine eligibility and enroll patients in health plans
    3. Analyze screening tool results and share with Care Team
    4. Link patients to services within Client and the broader community.
    5. Participate in staff team meetings, case conferences and training.
    6. Collect data and provides documentation in client files as required by agency and funders.
    7. Participate in obtaining deliverables required for various grants, such as, but not limited to, eligibility assessment, socioeconomic needs assessment, and medical disparities.
    8. Assist families with immediate crisis intervention, advocacy, problem-solving and other appropriate interventions, as needed
    9. Utilize de-escalation techniques in situations as appropriate
    10. Utilize program evaluation tools (i.e., pre/ post-tests, satisfaction surveys, etc.), analyzing and reporting on outcomes
    11. Provide resources for various health topics, such as nutrition, physical activity, hygiene, sexual health, and substance abuse.
    12. Maintain accurate and confidential records of interactions with students, families and other stakeholders.
    13. Work flexible hours, which includes some evenings and weekends
    14. Work at any of Client location, including clinics, mobiles, schools, homeless encampments, warming centers and patients’ homes, or patient appointments as assigned

    Secondary Responsibilities
    1. Attends workshops, training, and meetings as needed, and as requested.
    2. Performs other duties as assigned by supervisor.

    Qualifications:
    1. Required Education, Experience, and Training
    2. High school diploma or equivalent
    3. 1-2 years of experience in a community-based health care service capacity preferred
    4. Bilingual preferred
    5. CHW Certification - Recertification and/or retraining process, as required
    6. CEC Certification Preferred

    Work Conditions
    1. Travel to other sites and/or locations required
    2. May visit patient in their homes or accompany patients to their appointments
    3. Work outside in varying temperatures may be required
    4. Prolonged periods of standing or sitting.

    Physical Demands:
    1. This position requires:
    2. Sitting, walking, and standing in a typical medical/clinical office environment.
    3. Extended and/or prolonged periods of sitting, walking, or standing.
    4. Regular bending, stretching, and lifting up to 50 pounds.
    5. Regularly reaching with arms, climbing, balancing, stooping, kneeling, crouching, and crawling while performing job duties.
    6. Moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.
    7. Manual dexterity for tasks involving standard office equipment and computer keyboard, including repetitive movements such as typing.
    8. Traveling to work sites, including exposure to outside elements.
    9. Exposure to potentially dangerous materials, hazards, physical risks and situations, requiring adherence to extensive safety precautions and possibly the use of protective equipment.
