Job Summary
The Registered Nurse – Hospice Case Manager provides compassionate, patient-centered care to individuals and families in the hospice program. This role is responsible for coordinating and delivering nursing services, managing individualized care plans, and ensuring quality, comfort, and dignity for patients in end-of-life care. The Case Manager serves as the primary point of contact for patients, families, and interdisciplinary team members.
Key Responsibilities
- Assess patient needs and develop individualized care plans in collaboration with physicians and interdisciplinary team members.
- Provide direct nursing care, including symptom management, medication administration, and patient/family education.
- Coordinate hospice services across disciplines to ensure continuity of care.
- Serve as the primary case manager for assigned patients, monitoring progress and adjusting care plans as needed.
- Document patient assessments, interventions, and outcomes in compliance with regulatory and organizational standards.
- Support patients and families emotionally, offering guidance and resources during end-of-life care.
- Participate in interdisciplinary team meetings and contribute to care planning discussions.
- Ensure compliance with hospice policies, procedures, and applicable regulations.
Required Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of nursing experience, preferably in hospice, palliative care, or home health.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage multiple patients in a community-based setting.
- Proficiency in electronic medical records (EMR) systems.
Preferred Qualifications
- Prior hospice or palliative care experience.
- Experience in case management or care coordination.
- Knowledge of Medicare/Medicaid hospice regulations.
Certifications
- Basic Life Support (BLS) certification required.
- Hospice and Palliative Care Nursing Certification (CHPN) preferred.
