  • Job Summary
    The Registered Nurse – Long Term Acute Care provides specialized nursing care to patients with complex medical conditions requiring extended hospitalization. This role involves continuous monitoring, advanced clinical interventions, patient education, and collaboration with interdisciplinary teams to promote recovery, manage chronic illnesses, and ensure safe transitions of care.

    Key Responsibilities
    1. Deliver direct patient care including assessments, medication administration, and treatment procedures
    2. Monitor patient status and respond to changes in condition promptly
    3. Collaborate with physicians and interdisciplinary teams to develop individualized care plans
    4. Educate patients and families on chronic conditions, treatments, and discharge planning
    5. Maintain accurate documentation in the electronic medical record (EMR)
    6. Support rehabilitation efforts and promote patient independence
    7. Ensure compliance with hospital policies, safety protocols, and regulatory standards

    Required Qualifications
    1. Active Registered Nurse license in the state of practice
    2. Current BLS certification
    3. Current ACLS certification
    4. Minimum of 1–2 years nursing experience, preferably in acute care or LTAC settings

    Preferred Qualifications
    1. Experience in long term acute care or rehabilitation units
    2. Strong background in critical care and chronic illness management
    3. Certification in NIH Stroke Scale (NIHSS) or other specialty areas

    Certifications
    1. BLS required
    2. ACLS required
    3. NIHSS preferred
