    JOB TITLE: SLP Home Health

    JOB SUMMARY
    We are seeking a licensed Speech-Language Pathologist (SLP) with Home Health experience to provide essential rehabilitation services to patients in West Portland, OR, and its surrounding areas. This role involves providing direct patient care, developing and implementing individualized treatment plans, and collaborating with healthcare teams to ensure optimal patient outcomes.

    Key Responsibilities
    - Evaluate patients to assess speech, language, cognitive, and swallowing disorders.
    - Develop and implement individualized treatment plans tailored to patient needs and goals.
    - Provide skilled therapeutic interventions to improve communication, cognitive function, and swallowing abilities.
    - Educate patients and their families on strategies and exercises to support continued progress.
    - Document patient assessments, treatment plans, interventions, and progress notes accurately and thoroughly in the EMR system.
    - Maintain productivity standards and manage a caseload effectively.
    - Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
    - Ensure compliance with all healthcare regulations, licensing requirements, and facility policies.

    Schedule / Shift
    - Shift: 4 x 8-hour days per week.
    - This is a day shift position.

    Required Qualifications
    - Licensed Speech-Language Pathologist (SLP) in the state of Oregon.
    - Minimum of 1 year of skilled Home Health experience.
    - Must possess excellent communication and interpersonal skills.
    - Ability to work independently and manage time effectively.
    - Must have reliable personal transportation for home visits.
    - Must have a laptop for charting.

    Certifications
    - Basic Life Support (BCLS) certification required.
    - COVID-19 vaccination (primary series plus one booster) is required.

    Experience
    - At least 1 year of specialized experience in Home Health.
    - Experience with OASIS documentation is strongly preferred.

    Skills
    - Proficient in evaluating and treating a wide range of speech, language, cognitive, and swallowing disorders.
    - Skilled in developing and implementing evidence-based therapeutic interventions.
    - Strong documentation and charting skills.
    - Ability to adapt to varying patient needs and home environments.
    - Familiarity with home health best practices.

    EMR / Systems
    - Charting is performed using EPIC Remote Client for Home Health (on laptop) and Rover (phone app).
    - OASIS documentation is required.

    Patient Population
    - Home Health patients requiring speech-language pathology services.

    Unit / Specialty
    - Home Health

    Floating / On-Call
    - No floating between units required.
    - No on-call responsibilities.

    Additional Information
    - Productivity Standards: 30 productivity points are standard, potentially visiting up to 6 patients per day.
    - Mileage will be reimbursed at IRS standard rates.
    - Parking is free at the facility.
    - Business casual attire is expected; scrubs are allowed but not required (no specific color). Jeans are not permitted.
    - Facility can call off up to one shift per bi-weekly pay period.
