  • Job Summary
    We are seeking a dedicated Physical Therapist – Home Health to provide high-quality rehabilitative care to patients in their homes. This role involves evaluating patient needs, developing individualized treatment plans, and delivering therapy that promotes mobility, independence, and overall well-being. The ideal candidate will be compassionate, detail-oriented, and comfortable working independently in a home health setting.

    Key Responsibilities
    1. Conduct patient assessments to determine functional limitations and therapy needs.
    2. Develop and implement individualized treatment plans tailored to patient goals.
    3. Provide therapeutic interventions to improve mobility, strength, and independence.
    4. Educate patients and families on exercises, safety, and ongoing care strategies.
    5. Document patient progress and maintain accurate records using the Epic charting system.
    6. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
    7. Utilize laptop-based charting tools to maintain compliance with documentation standards.

    Required Qualifications
    1. Licensed Physical Therapist in the state .
    2. Strong clinical knowledge of rehabilitative practices and home health care.
    3. Proficiency in electronic charting systems (Epic preferred).
    4. Excellent communication and interpersonal skills.
    5. Ability to work independently and manage patient caseloads effectively.
    6. Must have at least 2 years of recent experience in the specialty within the past 2 years.

    Preferred Qualifications
    1. Prior experience in home health or community-based physical therapy.
    2. Familiarity with laptop-based charting and mobile healthcare technology.

    Certifications
    1. Active State Physical Therapist license (required).
    2. CPR certification (preferred).
