  • Job Summary  
    The Registered Nurse – Case Manager is responsible for coordinating patient care services to ensure quality outcomes and cost-effective treatment. This role involves assessing patient needs, developing individualized care plans, collaborating with interdisciplinary teams, and facilitating transitions of care. The RN Case Manager serves as a patient advocate, ensuring continuity of care across the healthcare continuum.

    Key Responsibilities
    1. Assess patient needs and develop comprehensive care plans.
    2. Coordinate services across multiple disciplines to ensure effective patient care.
    3. Monitor patient progress and adjust care plans as necessary.
    4. Facilitate discharge planning and transitions of care.
    5. Serve as a patient advocate, ensuring access to appropriate resources and services.
    6. Collaborate with physicians, nurses, social workers, and other healthcare professionals.
    7. Document case management activities in compliance with hospital policies and regulatory standards.
    8. Provide education and support to patients and families regarding care plans and available resources.

    Required Qualifications
    1. Current Registered Nurse (RN) license.
    2. Minimum of 3 years of nursing experience.
    3. Strong clinical assessment and critical thinking skills.
    4. Excellent communication and interpersonal abilities.
    5. Ability to work collaboratively with interdisciplinary teams.

    Preferred Qualifications (if any)
    1. Bachelor of Science in Nursing (BSN).
    2. Prior experience in case management, discharge planning, or care coordination.
    3. Familiarity with electronic medical record (EMR) systems.

    Certifications (if any)
    1. Basic Life Support (BLS) – required.
    2. Case Management Certification (CCM or ACM) – preferred.
