Job Summary
The Registered Nurse – Case Manager is responsible for coordinating patient care services to ensure quality outcomes and cost-effective treatment. This role involves assessing patient needs, developing individualized care plans, collaborating with interdisciplinary teams, and facilitating transitions of care. The RN Case Manager serves as a patient advocate, ensuring continuity of care across the healthcare continuum.
Key Responsibilities
- Assess patient needs and develop comprehensive care plans.
- Coordinate services across multiple disciplines to ensure effective patient care.
- Monitor patient progress and adjust care plans as necessary.
- Facilitate discharge planning and transitions of care.
- Serve as a patient advocate, ensuring access to appropriate resources and services.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals.
- Document case management activities in compliance with hospital policies and regulatory standards.
- Provide education and support to patients and families regarding care plans and available resources.
Required Qualifications
- Current Registered Nurse (RN) license.
- Minimum of 3 years of nursing experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work collaboratively with interdisciplinary teams.
Preferred Qualifications (if any)
- Bachelor of Science in Nursing (BSN).
- Prior experience in case management, discharge planning, or care coordination.
- Familiarity with electronic medical record (EMR) systems.
Certifications (if any)
- Basic Life Support (BLS) – required.
