Job Summary
The Nurse Practitioner provides comprehensive Primary and Family Care services, including assessment, diagnosis, treatment, and follow-up. This role manages acute and chronic conditions, supports preventive care initiatives, and delivers patient-centered education to promote wellness and self-care. The Nurse Practitioner works autonomously with minimal supervision while collaborating with the clinical care team to ensure continuity of care.
Key Responsibilities
- Provide primary and family care services, including patient assessment, diagnosis, treatment, and follow-up.
- Manage acute and chronic conditions using evidence-based clinical guidelines.
- Perform routine exams, health screenings, and diagnostic tests.
- Deliver health counseling, lifestyle education, and wellness support.
- Conduct patient triage, review medical histories, and manage treatment plans.
- Support preventive care initiatives, behavioral health screenings, and chronic disease management.
- Complete administrative duties such as clinic opening/closing, inventory management, insurance verification, documentation, and payment collection.
- Maintain a clean, safe, and compliant clinic environment.
- Collaborate with the clinical care team and coordinate with other healthcare professionals.
- Encourage patient engagement through education, self-care guidance, and adherence to treatment plans.
- Document accurately in EMR systems and maintain confidentiality in compliance with HIPAA standards.
- Participate in daily care team communications and contribute to quality improvement initiatives.
- Obtain DEA registration if required for prescribing.
- Provide care autonomously with minimal direct supervision.
Required Qualifications
- Master’s Degree from an accredited Family Nurse Practitioner (FNP) program.
- Current National Board Certification as an FNP.
- Active State Nurse Practitioner License in the state of employment.
- BLS Certification (active at time of hire).
- DEA registration required upon hire (unless otherwise noted).
Preferred Qualifications (if any)
- Minimum 1 year of recent Primary Care or Family Medicine experience as an NP preferred.
- Experience managing chronic disease conditions and familiarity with quality metrics.
- Candidates without Primary Care experience must complete the employer’s NP training program.
Certifications (if any)
- BLS Certification (required).
- DEA registration (required upon hire).
