Job Summary
We are seeking a dedicated RN Case Manager to coordinate and advocate for patient care. This role involves reviewing patient charts, meeting with families and healthcare providers, and developing the most suitable care plans. The RN Case Manager will maintain ongoing communication with patients throughout their care journey, ensuring they receive the support needed to focus on their health.
A successful RN Case Manager will alleviate logistical burdens, foster meaningful relationships with patients and families, and ensure continuity of care across all stages of treatment.
Key Responsibilities
- Conduct intake assessments with patients and families.
- Develop individualized treatment plans that consider physical, financial, and social factors.
- Adjust treatment protocols as needed to meet patient needs.
- Coordinate appropriate follow-up services to promote wellness.
- Ensure patients have reliable transportation to healthcare facilities.
- Collaborate with healthcare teams to monitor patient progress and determine next steps.
- Verify insurance coverage or financial resources for treatment.
- Provide innovative solutions or referrals for patients with limited access to care.
- Support families to feel equipped and confident in managing patient needs.
Required Qualifications
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
- Graduate of an accredited nursing program.
- Active Registered Nurse License (OR – single state).
- Basic Life Support (BLS) certification from the American Heart Association (AHA).
Preferred Qualifications
- Bachelor’s degree in Nursing or a related field.
- Additional experience in case management or patient advocacy.
Certifications
- Basic Life Support (BLS) from AHA (required).
