  • Job Summary  
    The Social Worker provides professional support and counseling services to patients and families, ensuring access to appropriate resources and care coordination. This role involves assessing psychosocial needs, advocating for patients, and collaborating with healthcare teams to promote overall well-being.

    Key Responsibilities
    1. Provide direct social work services to patients and families.
    2. Assess psychosocial needs and develop individualized care plans.
    3. Collaborate with physicians, nurses, and multidisciplinary teams to coordinate care.
    4. Advocate for patients to ensure access to community resources and support services.
    5. Document patient interactions and care plans accurately.
    6. Support patients and families in navigating healthcare systems and treatment plans.
    7. Maintain compliance with professional standards and organizational policies.

    Required Qualifications
    1. Licensed Social Worker.
    2. Minimum of 3 years of professional social work experience.
    3. Strong communication and interpersonal skills.
    4. Ability to work effectively in a multidisciplinary healthcare environment.
    5. COVID-19 vaccine required.
    6. Flu vaccine required.

    Preferred Qualifications (if any)
    1. Experience in hospital or healthcare settings.
    2. Familiarity with electronic medical record (EMR) systems.
