  • Job Summary  
    The Community Health Worker (CHW) I serves as a trusted member of the community and acts as a liaison between patients, healthcare providers, and community resources. This role supports outreach, engagement, care coordination, case management, patient education, and linkage to services such as food, housing, financial assistance, and transportation. CHWs actively participate in community events, clinics, and outreach programs to promote health equity and improve patient outcomes.

    Key Responsibilities
    1. Complete CHW Certification after 2,000 hours of work.
    2. Book appointments in the Electronic Health Record (EHR) according to provider scope.
    3. Use Microsoft Office to communicate and report information.
    4. Document patient encounters in EHR systems.
    5. Participate in health fairs, outreach events, and tabling to promote BACH services.
    6. Maintain logs and follow-up on no-show appointments.
    7. Schedule interpreters and transportation via patient health plans.
    8. Conduct outreach and enrollment in public benefit programs.
    9. Connect patients to internal and external resources to meet social and health needs.
    10. Serve as a liaison between the community and health/social services.
    11. Build and maintain appropriate relationships with patients, staff, and community partners.
    12. Support departmental documentation standards and adhere to clinic policies.
    13. Work flexible hours, including evenings and weekends, across multiple BACH locations (clinics, schools, mobile units, homeless encampments, warming centers, patient homes).

    Secondary Responsibilities
    1. Attend workshops, training, and meetings as needed.
    2. Perform other duties as assigned by supervisor.

    Required Qualifications
    1. High school diploma or equivalent.
    2. Two (2) years of experience in a community-based healthcare service capacity preferred.
    3. Bilingual skills preferred.

    Work Conditions
    1. Travel to other sites and community locations required.
    2. Work outside in varying temperatures may be required.
    3. Flexible hours, including evenings and weekends.
    4. Prolonged periods of standing or sitting.

    Physical Demands
    1. Regular sitting, walking, and standing in medical/clinical environments.
    2. Extended periods of physical activity including bending, stretching, lifting up to 50 pounds.
    3. Frequent reaching, climbing, balancing, stooping, kneeling, crouching, and crawling.
    4. Exposure to extreme temperatures, dust, fumes, odors, and loud noises.
    5. Manual dexterity for office equipment and computer use.
    6. Travel to work sites with exposure to outdoor elements.
    7. Adherence to safety precautions when exposed to potentially hazardous materials.
