JOB SUMMARY
Provide direct patient care in a Transitional Care Unit (TCU) setting, focusing on patients requiring ongoing monitoring and rehabilitation before discharge.

Key Responsibilities
- Assess, plan, implement, and evaluate patient care.
- Administer medications and treatments as prescribed.
- Monitor vital signs and patient status, reporting any changes promptly.
- Collaborate with physicians, therapists, and other healthcare professionals to develop and execute care plans.
- Educate patients and families on health management and discharge instructions.
- Document patient care activities accurately and thoroughly.
- Participate in team conferences and patient rounds.

Required Qualifications
- Registered Nurse (RN) license in the state of practice.
- Basic Life Support (BLS) certification.

Experience
- Previous experience in a Transitional Care Unit (TCU) or Med-Surg setting preferred.
- Experience caring for adult patients.
