JOB TITLE: Occupational Therapist

JOB SUMMARY
This position provides occupational therapy services to patients at Skyline Heights Nursing and Rehabilitation. The Occupational Therapist will assess, plan, and implement individualized treatment programs to improve patients' functional abilities and quality of life.

Key Responsibilities
• Evaluate patients to assess their physical, cognitive, and psychosocial status.
• Develop and implement individualized treatment plans to address patient needs and goals.
• Provide therapeutic interventions, including exercises, adaptive equipment training, and environmental modifications.
• Educate patients and their families on treatment plans, home exercise programs, and strategies for maintaining functional independence.
• Document patient progress, treatment provided, and outcomes accurately and in a timely manner.
• Collaborate with other healthcare professionals to ensure coordinated patient care.
• Monitor patient response to treatment and adjust interventions as necessary.
• Participate in discharge planning to ensure a smooth transition to home or another care setting.

Schedule / Shift
• Full-time position. Specific shift details to be discussed.

Required Qualifications
• Graduate of an accredited Occupational Therapy program.
• Valid state Occupational Therapist license.
• Current CPR certification.

Certifications
• CPR Certification

Experience
• Experience in a skilled nursing or rehabilitation setting is preferred.

Skills
• Proficiency in patient assessment and treatment planning.
• Strong knowledge of therapeutic techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong documentation skills.

EMR / Systems
• Experience with electronic health records is a plus.

• Adult and geriatric patients requiring rehabilitation services.

• Skilled Nursing and Rehabilitation

Floating / On-Call
• May be required to float to other units as needed.
• On-call duties may be assigned.
