Job Summary
We are seeking a compassionate and dedicated Medical Social Worker I to join our healthcare team. This entry-level role provides support to patients and families by addressing psychosocial needs, coordinating community resources, and assisting with discharge planning. The Medical Social Worker I collaborates with interdisciplinary teams to promote patient well-being, ensure continuity of care, and advocate for patients’ rights and access to services.
Key Responsibilities
- Conduct psychosocial assessments to identify patient and family needs.
- Provide counseling and emotional support to patients and families coping with illness, hospitalization, or end-of-life issues.
- Assist with discharge planning, including coordination of home health, rehabilitation, or community services.
- Collaborate with physicians, nurses, and other healthcare professionals to develop care plans.
- Educate patients and families about available resources, insurance coverage, and community programs.
- Document assessments, interventions, and referrals in compliance with hospital policies and regulatory standards.
- Advocate for patients’ rights and ensure equitable access to healthcare services.
- Participate in interdisciplinary team meetings and case conferences.
Required Qualifications
- Bachelor’s degree in Social Work (BSW) from an accredited program.
- Current state licensure or eligibility for licensure as a Social Worker.
- Strong communication, counseling, and intepersonal skills.
- Ability to work effectively with diverse populations in a healthcare setting.
- Knowledge of community resources and social service systems.
Preferred Qualifications (if any)
- Master’s degree in Social Work (MSW).
- Prior experience in a hospital, clinic, or healthcare environment.
- Familiarity with electronic health record (EHR) systems.
Certifications (if any)
- State licensure as a Licensed Social Worker (LSW) or equivalent.
- Certification in case management or palliative care (preferred).
