Job Summary
A Community Health Worker (CHW) at clent Health in San Jose acts as a vital bridge between the community and medical/social services.
The primary goal is to advocate for clients, improve health literacy, and facilitate access to essential resources, including healthcare, housing, and food security.
Key Responsibilities
Care Coordination: Connect clients with medical, dental, and behavioral health services.
Outreach & Navigation: Conduct home visits, accompany patients to appointments, and navigate them through complex healthcare systems.
Health Education: Provide culturally appropriate health and informational resources, chronic disease management coaching, and preventative health strategies.
Client Advocacy: Assist vulnerable populations with housing assistance, food insecurity, and other social service needs.
Documentation & Data Collection: Maintain accurate client records, track progress, and communicate clients' health concerns to the
broader healthcare team.
Required Qualifications
Typically requires a High School Diploma or GED. Higher education or certification in public health, social work, or a related field is a plus.
Previous experience in community outreach, case management, or working with underserved, low-income populations.
Bilingual proficiency (often English/Spanish or English/Vietnamese) is highly valued in the San Jose and Santa Clara County region.
Strong interpersonal communication, active listening, empathy, and cultural humility.
