  • Job Summary  
    The Occupational Therapist – Home Health provides patient-centered therapy services in the home setting. This role includes conducting assessments, developing individualized care plans, performing follow-up and discharge visits, and collaborating with physicians and care teams to ensure optimal patient outcomes.

    Key Responsibilities
    1. Conduct patient assessment visits to evaluate functional abilities and needs.
    2. Develop and implement individualized care plans tailored to patient goals.
    3. Complete follow-up and discharge visits to monitor progress and adjust care plans.
    4. Communicate effectively with physicians, nurses, and interdisciplinary care teams.
    5. Educate patients and families on therapeutic techniques to promote independence.
    6. Document patient evaluations, treatments, and progress in compliance with organizational standards.
    7. Utilize EPIC charting system to maintain accurate patient records.

    Required Qualifications
    1. Must have at least 2 years of recent experience in the specialty within the past 2 years.
    2. Current Occupational Therapist license (CAOT) in state.
    3. Basic Life Support (BLS) / CPR certification.
    4. Strong clinical assessment and treatment planning skills.
    5. Excellent communication and interpersonal abilities.
    6. Ability to work independently in a home health environment.

    Preferred Qualifications (if any)
    1. Experience with EPIC electronic medical record (EMR) system.
    2. Prior experience in patient education and family training.
    3. Advanced training in specialized therapy techniques.

    Certifications (if any)
    1. Occupational Therapist license (CAOT) – required.
    2. Basic Life Support (BLS) / CPR – required.
