Job Summary
The Occupational Therapist – Home Health provides patient-centered therapy services in the home setting. This role includes conducting assessments, developing individualized care plans, performing follow-up and discharge visits, and collaborating with physicians and care teams to ensure optimal patient outcomes.
Key Responsibilities
- Conduct patient assessment visits to evaluate functional abilities and needs.
- Develop and implement individualized care plans tailored to patient goals.
- Complete follow-up and discharge visits to monitor progress and adjust care plans.
- Communicate effectively with physicians, nurses, and interdisciplinary care teams.
- Educate patients and families on therapeutic techniques to promote independence.
- Document patient evaluations, treatments, and progress in compliance with organizational standards.
- Utilize EPIC charting system to maintain accurate patient records.
Required Qualifications
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
- Current Occupational Therapist license (CAOT) in state.
- Basic Life Support (BLS) / CPR certification.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently in a home health environment.
Preferred Qualifications (if any)
- Experience with EPIC electronic medical record (EMR) system.
- Prior experience in patient education and family training.
- Advanced training in specialized therapy techniques.
Certifications (if any)
- Occupational Therapist license (CAOT) – required.
