  • Job Summary
    The Social Worker provides professional support to patients, families, and communities by assessing psychosocial needs, developing care plans, and coordinating resources. This role promotes patient well-being, advocates for client rights, and collaborates with interdisciplinary teams to ensure comprehensive care.

    Key Responsibilities
    1. Assess psychosocial needs of patients and families to develop individualized care plans.
    2. Provide counseling, emotional support, and crisis intervention services.
    3. Coordinate resources by connecting clients with community services and referrals.
    4. Advocate for clients to ensure access to appropriate care and support.
    5. Collaborate with teams including healthcare providers, educators, and community organizations.
    6. Maintain documentation of case notes, assessments, and interventions in compliance with policies.
    7. Support program development and participate in community outreach initiatives.
    8. Promote patient rights including confidentiality, dignity, and respect.

    Required Qualifications
    1. Bachelor’s degree in Social Work (BSW) or related field.
    2. Licensed or certified as required by state regulations.
    3. Strong communication and interpersonal skills.
    4. Ability to manage multiple cases and prioritize effectively.
    5. Knowledge of community resources and social service systems.

    Preferred Qualifications
    1. Master’s degree in Social Work (MSW).
    2. Experience in healthcare, mental health, or community service settings.
    3. Familiarity with electronic medical records (EMR) systems.

    Certifications
    1. State licensure or certification in Social Work required.
