  • Job Summary  
    We are seeking a compassionate and skilled Registered Nurse (RN) to provide comprehensive care in a community setting in Fremont. This role involves patient assessment, triaging, medication management, and chronic disease management, with a focus on serving diverse populations.

    Key Responsibilities
    1. Clinical Care: Deliver routine nursing care, administer medications, perform medication reconciliation, and conduct patient assessments.
    2. Triage: Evaluate patient symptoms to determine urgency and prioritize care, particularly in triage nurse positions.
    3. Documentation: Maintain accurate and timely electronic health records (EHR) and patient documentation.
    4. Patient Education: Provide education on health conditions, preventive care, and treatment plans.
    5. Case Management: Coordinate patient care, monitor health status, and assist with individualized care plans.

    Required Qualifications
    1. Active Registered Nurse (RN) license in the state of California.
    2. 1–2 years of clinical nursing experience.
    3. Proficiency with Electronic Health Records (EHR).
    4. Strong clinical assessment skills and ability to work independently.
    5. Effective oral and written communication skills for patient interaction and team collaboration.

    Preferred Qualifications (if any)
    1. Experience in community health or outpatient care settings.
    2. Bachelor of Science in Nursing (BSN).

    Certifications (if any)
    1. Basic Life Support (BLS) certification required.
    2. Advanced Cardiac Life Support (ACLS) certification preferred.
