Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to provide comprehensive care in a community setting in Fremont. This role involves patient assessment, triaging, medication management, and chronic disease management, with a focus on serving diverse populations.
Key Responsibilities
- Clinical Care: Deliver routine nursing care, administer medications, perform medication reconciliation, and conduct patient assessments.
- Triage: Evaluate patient symptoms to determine urgency and prioritize care, particularly in triage nurse positions.
- Documentation: Maintain accurate and timely electronic health records (EHR) and patient documentation.
- Patient Education: Provide education on health conditions, preventive care, and treatment plans.
- Case Management: Coordinate patient care, monitor health status, and assist with individualized care plans.
Required Qualifications
- Active Registered Nurse (RN) license in the state of California.
- 1–2 years of clinical nursing experience.
- Proficiency with Electronic Health Records (EHR).
- Strong clinical assessment skills and ability to work independently.
- Effective oral and written communication skills for patient interaction and team collaboration.
Preferred Qualifications (if any)
- Experience in community health or outpatient care settings.
- Bachelor of Science in Nursing (BSN).
Certifications (if any)
- Basic Life Support (BLS) certification required.
- Advanced Cardiac Life Support (ACLS) certification preferred.
