Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health and Hospice team. The RN will provide comprehensive nursing care to patients in their homes, focusing on comfort, dignity, and quality of life. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating with interdisciplinary teams.
Key Responsibilities
- Deliver patient-centered nursing care in home and hospice settings.
- Conduct initial and ongoing assessments to develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Provide symptom management and palliative care for patients with chronic or terminal illnesses.
- Educate patients and families on disease processes, treatment options, and end-of-life care.
- Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic care.
- Document patient assessments, interventions, and outcomes accurately in the electronic health record system.
- Offer emotional support and guidance to patients and families during challenging times.
- Ensure compliance with regulatory standards, agency policies, and professional nursing practices.
Required Qualifications
- Current, active RN license in the practicing state.
- Graduate of an accredited nursing program (BSN or ADN).
- Minimum of 2 years of nursing experience, preferably in home health or hospice care.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently in diverse home environments.
- Excellent communication and interpersonal skills.
Preferred Qualifications (if any)
- Prior experience in palliative care or hospice nursing.
- Familiarity with electronic health record (EHR) systems.
- Advanced training in pain management or end-of-life care.
Certifications (if any)
- Basic Life Support (BLS) certification.
- Hospice and Palliative Care Nursing Certification (CHPN) preferred.
